Primary Care?

Direct Primary Care (DPC) is a new approach to Primary Care that seeks to bring the physician-patient relationship back to its core. Imagine a doctor who actually has the time to get to know you and become a trusted resource to your family, understand your health goals and help you achieve them.

In following with our model of transparency in pricing our patients pay a low monthly membership fee in exchange for direct access to their doctor.

No patient wants to be just another rushed 10 minute appointment. By limiting the number of patients on each doctor’s panel to <500, the doctor is able to spend more quality time with patients, it’s a win-win. If you or your loved one gets sick don’t stress we will work to see you in the same day or next day, this includes phone/video appointments if needed. By limiting the number of patients we don’t have to sacrifice quality of care.

Freedom Doctors has opted not to accept insurance because it causes unnecessary barriers on the doctor-patient relationship. Typical insurance-driven care is inherently not user-friendly. Our goal through our concierge style, Direct Primary Care (DPC) practice is to recenter the relationship between the doctor and patient, no middlemen involved.

Monthly Subscription Fees

Children 0-23 months old (with a parent)

  • Please call to discuss pricing for children <2 year of life, children <2 must be enrolled with parent/guardian

  • Well child appointments in the first 2 years of life include after hospital discharge, 1 week, 1 month, 2 month, 4, 6, 9, 12, 15, 18, 24 months of life

Children 2-17 year old (with a parent)

  • $50 / month

  • $100 / month (without a parent)

Adults 18-44 years old

  • $90 / month

Adults 45-64 years old

  • $109 / month

Adults 65+ years old

  • $130 / month

Monthly subscription includes:

  • Initial Meet & Greet visit

  • Annual physical / Well child visits

  • Home visits: Up to 4/year per person (think you’ll need more? ask about additional home visits)

  • Virtual care: Unlimited

     

One time enrollment fee of $199 per person OR $250 per family charged once at the time of your first monthly payment.

There is re-enrollment fee if you choose to re-join the practice after leaving.

FAQs

  • Get to know us. We offer complimentary meet & greet appointments for future patients to ensure it will be a good patient-physician fit.

  • Our practice is intentionally small and local. Submit your information in our contact form or give us call and we can see if we can accommodate your location, additional travel fees may apply.

  • If it's a true emergency, call 911, or go to the emergency room.

    If it's something urgent but not life threatening, call us first. We may be able to see you right away, and save you a trip to the Emergency Room or to the Urgent Care.

  • If you need a specialist, we can refer you. If you want to use insurance for specialist services, you can do so. We will help coordinate care with your specialists as needed.

  • Ours hours are similar to your normal doctor’s office, but we try our best to accommodate urgent after hour and weekend calls for members.

  • We do not bill insurance for physician-related fees. Our monthly membership fees allows us to provide high-quality, personalized, and affordable care for you and your family

  • Yes. Insurance is recommended and/or required in many states. Think “catastrophic medical coverage”. It is important to note, though, that the direct primary care model is not insurance and does not meet the criteria for insurance as set forth in the Affordable Care Act.

  • Patients join the practice after an initial meet & greet appointment to ensure a good fit for patient/doctor and payment of a one-time individual/family registration fee.

  • 30 days written notice is required to cancel a membership. Please keep in mind that if you wish to re-enroll after canceling a membership there will be a $199 re-enrollment fee. Re-enrollment is also subject to availability as there are a limited number of patients that can be accepted into the practice.
